What to Bring With You

1. Picture ID (i.e. driver's license, school ID, military ID, etc.)
2. NEW insurance card(s) if it has changed
3. Co-payment (cash, check, or credit card)
4. Actual bottles, if available, of any NEW medications you are taking since your last visit (preferred) – otherwise a list of new medication(s) with dosage and frequency
5. Hospital follow-up: Please bring any records from the hospital
6. Update your address, phone number, and preferred pharmacy during check-in
7. Give the staff any new medical records/labs that you have and/or forms that need to be completed

Dr. Tui

Dr. Srisawat, or "Dr. Tui" as she is affectionately known by her patients and staff, was born in Washington but spent most of her childhood in Thailand and then California. She graduated medical school from Loma Linda University in Loma Linda, California. She then went on to complete her family residency training at Florida Hospital in Orlando, Florida. Dr. Tui is board certified in family medicine and is a member of the Florida Academy of Family Physicians and the American Academy of Family Physicians. As part of her current practice, she also serves as a clinical preceptor for Florida State University medical students in the areas of geriatrics and chronic care. During her free time Dr. Tui enjoys reading, cooking, and traveling.
